Annual On-Campus Flu Clinic
The annual Friends and Family Flu Clinic at Poly will be held on Friday, September 15, from 7:30 a.m. - 1 p.m. in the Multi-Purpose Room above Gamble Gym.
Please plan to bring the following to the clinic:
- Consent form (printed out) - click here
- Copy of your insurance card
There is a cost of $44 for the flu shot for family and friends if Vons cannot bill your insurance because you are either uninsured or have Kaiser insurance. Unfortunately, Kaiser does not reimburse Vons for flu shots.
Students are welcome to attend the Flu Clinic if a parent accompanies them and will not miss class.
