This is a required form for only those students who either self-administer prescription medication or whose parent or guardian is authorizing the school nurse to administer a prescribed medication. Please note that the summer program requires the original, hard copy to be turned into the Summer Program Office. If you have questions, please contact Director of Health Services Marcy Kwitny.
We offer an early drop-off time that begins at 7:45 a.m (at no additional cost) for those students signed up for an A period (8:30 am - 9:30 am), and we offer after-hour care for $160 per session. After hour care is offered to those students who have an F period (3 pm - 4 pm) but who need to stay longer. Our after hour care starts at 4 pm and ends at 6 pm.
Visit the registration site and create a username and password. Once you input your child's information (name, grade, date of birth), you can view all the available classes, as well as those classes that have been waitlisted, which you can also add to your registration.
You can pay online by simply logging into the registration site and click the orange "Polytechnic Summer Program 2018" link on the "home" screen, under the "View My Registrations" field. This will take you to the Camp Registration page that has your registration details. Under the Financial area, click the orange button that says "new payment." This will take you to the online payment page. You may also call the PolySummer Office at 626-396-6307.
The online registration is configured to hold your registration for 40 minutes. If, after 40 minutes, you do not complete your registration, those seats in the classes are released to other families. Be sure to complete your registration within 40 minutes to get the classes you selected.
