Information for Parents of Rising Middle and Upper School Students Regarding Voluntary Health Screenings

 
This Spring, Poly students currently in grades 5-11 will have the opportunity to participate in a voluntary health screening that will be administered at Exer Urgent Care in Pasadena. Exer Urgent Care is affiliated with Huntington Hospital and is staffed with the same ER doctors who work at Huntington Hospital. The health screening will meet the school requirement for an annual physical for students entering grades 6-12 in the Fall. Both the physical exam and the completion of the school's Physician Consent Form will be provided to Poly families at NO COST. 
 
Exer Urgent Care will host two Poly screening days at their facility (3160 E. Del Mar Blvd., Suite 210, Pasadena) at the following times: Wednesday, April 15, from 3:00pm- 7:00pm, and Saturday, April 18, from 10:00am to 3:00pm. To take advantage of this opportunity, please sign up online for a time slot (three students per 15-minute time slot).
 
The health screening program is entirely voluntary and is not required to participate in Poly athletics. You may take your child to their primary care physician for the annual physical exam.
