Injury Report Form

Please report all injuries to the Health Office. An Injury Report form must be completed and filed with the Health Office in the event of a school-related injury involving a student, employee, or visitor to the School. The injury report should be completed within 24 hours by the person who initially attends to the injured or by the person who witnesses the circumstances at the time of the injury.
